About the Role:

As an LPN at The Gardens at Country, you will be responsible for providing high-quality care to our residents, including administering medications, performing basic nursing tasks, and monitoring vital signs. You will also have the chance to build positive relationships with our residents and their families, and contribute to a collaborative and supportive care team.

Responsibilities:

• Administer medications and treatments as prescribed by physicians

• Perform basic nursing tasks, such as taking vital signs and assisting with activities of daily living

• Monitor and report resident changes and concerns to the nursing team

• Develop and implement individualized care plans in conjunction with the interdisciplinary care team

• Communicate effectively with residents, families, and healthcare providers

• Participate in quality improvement initiatives and maintain accurate records
