Job Description
Title: Physical Therapist

Schedule:
- Days: 8 hours
- Shift: 8 hr days

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements:
- 1 year of Physical Therapy experience (required)
- LTC or Rehabilitation experience (required)
- Massachusetts State License (required)

Roles & Responsibilities:
- Screen, evaluate, and rehabilitate patients with physical function disorders and/or wound care.
- Develop and implement appropriate plans of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Assess patient progress and adjust treatments as necessary while documenting in clinical records.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Communicate effectively with healthcare team members regarding patient progress and treatment plans.
- Document all services provided according to regulatory and clinical requirements, ensuring proper record maintenance.
- Participate in required meetings and training sessions, including patient care conferences and rehabilitation meetings.

Compliance / Notes:
- Must adhere to APTA and state-required practice acts, and ethical standards in professional practice.
