**This is an In Office/On Site position , Monday through Friday and requires on call rotation**

This role operates administratively in a variety of different Service Departments . Rather than institutionalizing individuals who have these physical and mental health needs, we provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.

As a core team member, you’ll be responsible for monitoring services, creating new strategies to reach goals, and coordinate services in a way that empowers people to meet and exceed their goals. • Team Leadership & Culture: Build and maintain a positive team culture through active staff engagement, regular team meetings, coaching, and ongoing support. Participate in on-call responsibilities and provide short-notice coverage when needed to maintain continuity of services.
• Resource & Operational Management: Oversee key resources and operational needs, including vehicle scheduling, individual budgets, representative payee coordination, medication check-ins, appointment scheduling, transportation, and other supports necessary for effective service delivery.
• Person-Centered Service Coordination: Coordinate with individuals, families/guardians, case managers, Medicaid Community Based Care Managers (CBCM), and the Interdisciplinary Team (IDT) to develop individualized plans, establish goals and outcomes, maintain authorizations and required assessments, and monitor individual progress.
• Compliance & Documentation: Ensure services comply with applicable agency policies, laws, regulations, and industry standards. Maintain complete, accurate, and timely documentation in the approved EMR, including service records, incident reports, investigations, individual interactions, goal progress, and changes to services or rights.
• Advocacy & Community Partnerships: Advocate for individuals to have meaningful choice, independence, dignity, and control over their lives while ensuring services reflect their needs, preferences, and goals. Collaborate with internal teams and community partners to strengthen resources, relationships, and opportunities available to individuals served.

• Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this.
• Educational Requirement: Minimum Education Requirement of High School Diploma/GED equivalent.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

• May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
• May require prolonged sitting and working at a computer for up to eight hours.
• May require frequent bending, stooping, and stretching.
• May require prolonged standing for up to eight hours.
• Must wear hearing and eye protection in required areas.
• Must have ability to see objects/persons at a distance.
• Must have ability to see close work such as typed or handwritten material.
• Must have ability to hear conversations in a quiet and a noisy environment.
• Must have ability to determine where a sound is coming from.
• Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
• Must have ability to communicate through speech.
• Must have ability to drive and transport for service provision as needed.
• Must remain awake during working hours, where applicable.
