Through compassion, clinical expertise, and meaningful connections, our nurses help create an environment where residents feel safe, respected, and cared for.If you are looking for a nursing career where your knowledge, compassion, and dedication make a lasting impact, apply today.ResponsibilitiesRegistered Nurses at Bear Mountain provide skilled, compassionate care while helping residents maintain their dignity, independence, and quality of life.As an RN, you will:Resident-Centered Nursing CareProvide direct nursing care to residents utilizing the nursing process of assessment, planning, intervention, implementation, and evaluationComplete comprehensive assessments and monitor changes in resident conditionDevelop, implement, and evaluate individualized care plans in collaboration with residents, families, and interdisciplinary teamsAdminister medications and treatments safely and accurately according to physician orders and facility policiesMonitor and document vital signs, clinical observations, treatments, and resident responsesAssist residents with activities of daily living while promoting independence, dignity, and self-esteemRecognize and respond to changes in physical, emotional, cognitive, and psychosocial needsSupport residents with chronic conditions, rehabilitation needs, and complex healthcare challengesClinical Collaboration & CommunicationCollaborate with physicians, nursing leadership, rehabilitation teams, and other healthcare professionals to support resident outcomesCommunicate effectively with residents, families, and care team membersEducate residents and families regarding care needs, treatment plans, and health managementPromote resident participation in their healthcare decisions whenever possibleMaintain accurate, timely, and complete clinical documentationQuality Care & Professional PracticeFollow all federal, state, and facility regulations, policies, and proceduresMaintain resident confidentiality and uphold HIPAA standardsParticipate in quality improvement initiatives and support Bear Mountain's QAPI practicesMaintain required education, certifications, and professional competenciesFollow safety standards and infection prevention practicesSupport a positive, respectful, and team-focused work environmentQualities as a Licensed Registered Nurse (RN) with a current State license required.Registered Nurse (RN) Training and work experience in a geriatric setting preferred.Ability to supervise and work well with other personnel required.Qualities of maturity, diplomacy, and ability to work well with others required. Caring, respectful attitude towards residents required.Physical stamina for constant activity required.Ability to perform tasks to established standards of excellence required.Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Teamwork, and Passion for Excellence and Reverence for the Individual is required.This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. We provide Short-Term Rehabilitation and Long-Term Care services with specialized support including hospice care and respite care.
