- Shifts will be 8-12 hours and candidates will be needed for full and part time.
- LPN's observe, measure, and record clinical data, which is used by other health practitioners to make health care decisions.
- LPN's can pass medications, complete wound care, administer vaccinations and execute medical regimens as prescribed by NY Licensed Physicians, Dentists, Nurse Practitioners, Physician Assist and Podiatrists.
- LPN's identify health problems or changes in a person's health status and determine whether immediate intervention is needed.
- LPN's must report these problems or changes in a timely manner to an RN or directing practitioner.
- LPN's are not legally allowed to take independent action to address health problems or changes.
- Medication administration and tube feeding via gastrostomy or jejunal tube observation
- Provides personal care and maintains living areas, including making beds, changing linens, lifting and moving individuals, and safeguarding personal belongings.
- Current, valid, and unrestricted NY license/certification in good standing, as required for the position.
- Candidates should be experienced with developmental disabilities,
- Administration of medications/including Insulin; G-Tube/J-Tube feedings; Respiratory treatments, Tracheotomy care/Suctioning, etc.
- Minimum of one (1) year/s of recent experience as an LPN .
- Additional certifications may be required.
- Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test, or other client-specific requirements ) .
