Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our growing healthcare team in Danbury, CT!!
Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care.

Compensation and Benefits:
- Earning potential: $115,000$200,000 (salary based on experience + easily achieved monthly bonuses)
- Flexible Schedule: Monday Friday. No on-call, weekends, or holidays.

Key Responsibilities
- Deliver person-centered care using evidence-based practices
- Collaborate with interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Compliance with facility, state and federal regulations/protocols.
Qualifications
Qualifications:
- Active license as a Nurse Practitioner (NP)
- Minimum 2 years of post-certification experience
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
