Registered Nurse (RN) - Home Health

We are seeking a compassionate Registered Nurse (RN) for a home health role supporting in-home patient visits, telehealth consultations, and care coordination. Responsibilities Include Conducting thorough home safety evaluations and physical assessments. Delivering clinical interventions such as medication administration, wound care, and point-of-care testing under provider supervision. Maintaining accurate patient records and documentation. Providing patient and caregiver education. Supporting care transitions, end-of-life discussions, and quality improvement initiatives. Collaborating with interdisciplinary teams to develop and implement personalized care plans. Participating in team meetings and building community relationships. Adhering to regulatory standards and organizational policies. Qualifications Include Active state RN licensure (compact preferred). Minimum 3 years of direct patient care experience; hospice, palliative, or geriatric experience preferred. CPR certification and valid driver’s license with auto insurance. Proficiency with electronic health records, Microsoft Office, and telehealth platforms. Strong communication, organizational, and decision-making skills. Ability to work independently, lift up to 20lbs, and travel locally as needed. This role does not include supervisory duties and involves working primarily in home settings to provide high-quality, patient-centered care.
