Case Manager RN - Hospital-at-Home Transitions Lead
Tampa General Hospital
Tampa General Hospital is seeking a Case Manager RN to ensure safe and timely patient transitions through the acute care process. This role includes collaborating with interdisciplinary teams to develop and monitor care plans, and addressing clinical and psychosocial barriers to progressing care. The ideal candidate has an Associate's Degree in Nursing, active RN licensure, and substantial nursing experience, particularly in Case Management or Emergency Medicine. This full-time position is based on-site at TGH Main Campus in Tampa, Florida. #J-18808-Ljbffr
