In this role, you will deliver skilled nursing focused on symptom management, comfort, and quality of life for patients with serious or terminal illness. You will complete assessments, create and update care plans, educate patients and families, and collaborate closely with an interdisciplinary team. This mission-driven position offers opportunities to grow your clinical skills while supporting patients and families with dignity and respect at end of life. Responsibilities • Provide skilled hospice nursing care in patient homes and facilities, focusing on comfort and symptom management • Complete comprehensive nursing assessments and develop individualized care plans • Administer medications, treatments, and interventions per hospice and palliative care protocols • Educate patients and families on disease process, medications, and end-of-life care options • Coordinate with physicians, social workers, chaplains, CNAs, and other interdisciplinary team members • Monitor patient condition, update care plans, and document care accurately in the EMRParticipate in care conferences and communicate changes in condition to the care team promptly • Support patients and families emotionally, providing guidance and resources for advance care planning • Ensure compliance with hospice regulations, policies, and best-practice standards • Participate in on-call rotation and respond to urgent patient needs as assigned Required Skills • Hospice nursing • Palliative care • Pain and symptom management • IV therapy and medication administration • Wound care • Electronic Medical Records (EMR) documentation • Care planning and case management • End-of-life patient and family education • Interdisciplinary team collaboration • Regulatory and hospice compliance knowledge## Additional DetailsCity: RamseyState: IllinoisCountry: US
