How you'll Contribute Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process. Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues Play a key role in the discharge planning process to support safe, successful transitions Why join us We believe that investing in our employees is the first step to providing excellent patient care. What we're looking for Applicants should possess the following qualifications and skills: Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program. Current state licensure as Licensed Practical Nurse. CPR/BCLS certification. ACLS preferred. Minimum six months' Medical/Surgical experience in an acute care setting preferred. Excellent oral and written communication and interpersonal skills.
