Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services. Required Qualifications Current Registered Nurse license valid for practice in Missouri Current driver's license Two years of Registered Nurse experience in a clinical care setting One year of full-time hospice experience Reliable transportation and valid automobile liability insurance Ability to work independently and make sound clinical decisions Strong written, verbal, organizational, and interpersonal skills Key Responsibilities Coordinate and oversee all aspects of new patient admissions Collaborate with patient care teams to support seamless communication and continuity of care Educate patients and families regarding condition, needs, and care Perform head-to-toe physical assessments Complete all required documentation accurately and on time Perform other duties as assigned Comprehensive medical, dental, vision, and supplemental benefit options; disability coverage options; employer-paid life insurance; HSA/FSA options; mileage reimbursement; generous PTO and paid holidays. #J-18808-Ljbffr H.
