Registered Nurse - Case Management

This is a 13-week contract position.

5x8-Hour Days (Monday - Friday, 08:30 - 17:00) - NO EXCEPTIONS. No Call required.

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of case management, utilization review, and discharge planning.

Manage comprehensive case assessments and discharge planning for patients.

Coordinate care to ensure continuity and optimize patient outcomes.

Conduct utilization review and implement evidence-based practices.

Adapt to frequent changes while ensuring compliance with regulatory and departmental guidelines.

Minimum of 4 years of experience required in a healthcare setting as a Registered Nurse.

Graduate from an accredited school of nursing; BSN preferred.

Two (2) years of healthcare experience specifically in case management.

Epic experience is required.

Licensed to practice as a Registered Nurse in North Carolina.

BLS certification is required; ACM/CCM certification preferred.

No first-time travelers or locals within a 50-mile radius will be considered.
