Requirements:

• Assess and monitor patient conditions and rehabilitation progress.
• Administer medications, treatments, and therapeutic interventions.
• Support rehab goals through patient education and coordinated care.
• Document patient status and collaborate with therapy and medical teams.
• Assist with discharge planning and patient/family education.
