Acute Care RN Care Manager: Discharge & Utilization Lead

TAG MedStaffing is seeking an experienced RN Care Manager to join a collaborative acute care team focused on care coordination, utilization review, and safe discharge planning. In this role you will complete comprehensive patient assessments, identify discharge planning needs from admission through discharge, and coordinate multidisciplinary care with physicians, nurses, social workers and community providers to ensure smooth transitions. #J-18808-Ljbffr TAG MedStaffing
