What You'll Do

• Perform skilled nursing visits in patients' homes.
  
• Provide wound care, medication management, and ongoing patient assessments.
  
• Educate patients and caregivers to promote independence.
  
• Communicate changes in condition with the RN Case Manager and healthcare team.
  
• Complete accurate and timely documentation.
  
• Help patients remain safe, healthy, and comfortable at home.
  

What We're Looking For

• Active Minnesota Licensed Practical Nurse (LPN) license.
  
• Reliable transportation, valid driver's license, and current auto insurance.
  
• Ability to work independently and manage a daily field schedule.
  
• Previous experience in skilled nursing, rehabilitation, TCU, hospice, or home health is helpful but not required.
  
• Compassionate clinicians who enjoy building relationships with patients.
