The Opportunity Seeking Board Eligible/Certified Family Practice or Emergency Medicine trained physicians. Current IN state license is a plus. The Practice Community Hospital Outpatient Center Schererville, Indiana Community Hospital Outpatient Center, St. John St. John, Indiana These outpatient centers serve the local community, both appointments and walk-ins. Additional services include heart care, immediate care, laboratory and therapy services, among others. Minor procedures performed at both locations. Single provider coverage. Will be cross-credentialed at both sites. Typical hours of operation are Monday through Friday, 8am-8pm, and Saturday and Sunday, 8am-4:30pm.
