Job Title Contingent Clinical Care LVN/LPN - MC Address Hoboken, NJ Job Description Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources. Conduct medication reviews to identify potential side effects and drug interactions. Document patient medical history using electronic health records. Critical Information Schedules created in 6‑week blocks following 3 weeks of training/orientation.
