Outpatient Clinic Registered Nurse

Community Care Physicians is seeking a Registered Nurse to join our multispecialty, physician-owned medical group in Latham, NY. In this outpatient role, you will deliver patient-centered nursing care through assessments, medication administration, and care coordination across primary and specialty services. The RN will educate patients and families, support chronic disease management, and ensure accurate EHR documentation. You’ll collaborate closely with physicians and a multidisciplinary team, participate in quality improvement initiatives, and help advance our mission of community-focused, high-quality care while enjoying a supportive, work–life–balance–oriented environment.
