Population Health Dietitian: Food Rx & Diabetes Education

MedStar Health is seeking a Population Health Dietitian to join the Food Rx program in Baltimore, MD. The role focuses on nutrition counseling, diabetes education, and medically tailored groceries to help patients with chronic diseases and food insecurity. This PRN position offers 2-3 days per week, collaborating with community health advocates and providers in the Collaborative Care Program to identify eligible patients and coordinate care plans.
