Providence Saint John's Health Center in Santa Monica, CA is hiring a full-time Care Manager RN. The Inpatient Care Manager delivers patient-centered management for at risk patients in an acute care environment, including assessment, planning, coordination of care, discharge planning and documentation of interventions. This role requires an Associate's Degree in Nursing and California RN license, with 2 years of acute care experience and 1 year in care management or utilization review; TIPS or #J-18808-Ljbffr Providence Health & Services
