We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health services, conduct patient assessments, develop individualized care plans, and deliver exceptional patient-centered care. This is an excellent opportunity for a nurse who values independence, flexibility, and meaningful patient relationships.
What You'll Do
- Perform home health nursing visits and patient assessments
- Develop and manage individualized care plans
- Provide wound care, medication management, disease management, and patient education
- Complete OASIS documentation and maintain accurate clinical records
- Collaborate with physicians, caregivers, and interdisciplinary teams to ensure quality outcomes
Qualifications
- Active Texas RN license
- Minimum 1 year of nursing experience
- Home Health experience preferred
- OASIS experience or willingness to learn
- Strong clinical assessment, communication, and documentation skills
- Valid driver's license and reliable transportation
- Current CPR certification (or ability to obtain)
Preferred
- 2+ years of Home Health experience
- Experience with WellSky/Kinnser or similar EMR systems
- Knowledge of Medicare guidelines and regulations
- Bilingual (English/Spanish) a plus
