This leadership role includes staff supervision, caseload management, compliance with regulatory requirements, and coordination with physicians, hospitals, and community partners. Key responsibilities include:Supervise and support clinical staff, including RNs, LPNs, therapists, and home health aidesManage patient care services, including OASIS documentation, care plans, and physician communicationOversee staff caseloads, scheduling, and performance evaluationsEnsure accurate, timely, and complete clinical documentation within the EHR systemParticipate in patient case conferences, team meetings, and interdisciplinary care planningProvide leadership for onboarding, staff education, and ongoing professional developmentConduct chart audits and monitor quality metrics to achieve positive clinical outcomesServe as liaison with referral sources, hospitals, and community agencies for care coordination and discharge planningParticipate in on-call clinical manager rotation and provide problem-solving support during off-hours as neededPromote staff engagement, team collaboration, and patient satisfaction in alignment with Andwell’s missionWhat You’ll Bring:Graduate of an accredited nursing school; BSN preferred or actively enrolled in a BSN programCurrent RN license in the State of Maine (required)Minimum of 3–4 years of nursing experience in acute care with at least 1 year of leadership experience; home healthcare management experience strongly preferredCurrent CPR certification (Healthcare Provider level)OASIS knowledge and certification (or willingness to obtain within 2 years)Strong leadership, communication, and organizational skillsKnowledge of reimbursement mechanisms, community resources, and interdisciplinary care modelsAbility to analyze data, monitor quality outcomes, and support staff performance improvementValid Maine driver’s license, reliable vehicle, and auto insuranceAndwell Health Partners is an equal opportunity employer.
