Schedule: Monday-Friday, 8:00 AM - 5:00 PM (Holidays and Weekends as needed)

Key Responsibilities

As a Hospice RN Case Manager , you will:

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
• Deliver holistic patient care , addressing physical, emotional, psychological, social, and spiritual needs
• Collaborate with the interdisciplinary care team , including physicians, social workers, and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update individualized plans of care
• Provide end-of-life care , including clinical pronouncement and family support
• Maintain accurate, timely documentation in compliance with state, federal, and company standards
• Supervise and oversee care delivery by Home Health Aides (HHAs)

Qualifications

• Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of quality assurance and hospice care standards
• Valid driver's license and auto insurance
