Position Overview The Illinois Department of Public Health is seeking a highly motivated, detail‑oriented individual to serve as a Health Facilities Surveillance Nurse in the Division of Intermediate Care Facilities/Individuals with Intellectual Disabilities. This individual will conduct surveys of facilities, including state licensure, federal certification and complaint surveys, and will prepare written survey documentation. Under general supervision, the HFSN will surveys facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures. Responsibilities include traveling to conduct routine health and safety surveys, interviewing facility residents, staff, administrators and health care providers, observing patient care, reviewing medical records and facility documents related to licensure and regulatory compliance, and performing monitoring functions at non‑compliant facilities. Requires licensure as a Registered Nurse in the State of Illinois. Preferred Qualifications One (1) year experience in ICF/IID setting. Three (3) years professional nursing experience. Two (2) years experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow‑ups, complaints and incident investigation surveys. One (1) year experience preparing complex written & oral reports. One (1) year work experience in Microsoft Word, Microsoft Outlook, Excel, PowerPoint, Smartsheet. One (1) year experience exercising judgement, discretion and maintaining confidentiality during the conduct of the survey process. Two (2) years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders. Conditions of Employment Requires appropriate, valid driver’s license. Requires ability to travel in performance of duties with overnight stays as appropriate. Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last‑minute overtime.
