Position Overview

This contract assignment is for a Registered Nurse (RN) in Home Health. The standard duration is 13 weeks, with a typical schedule of Monday through Friday, 8 AM to 5 PM.

Job Responsibilities

Provide home visits to patients, attending periodic office meetings.
Manage a caseload of approximately 58 patients, with a weekly productivity expectation of 30 points.
Types of visits may include Start of Care, Wound Care, Follow-up visits, and other adult home health services.
Documentation of visits must be completed within 24 hours.
Travel to patients' homes and the clinic for weekly meetings, with an expected travel distance of up to 75 miles.

Work Environment

This position involves on-call responsibilities and weekend rotations, with at least one weekend and several weekdays on-call each month. Holidays may also require availability around Thanksgiving, Christmas, and New Year's.

Qualifications

Minimum of one year of recent adult home health or hospice experience required.
Registered Nurse (RN) license must be active and in hand.
CPR and BLS certifications are required.
Proficiency in Homecare Homebase (HCHB) is required, with usage within the past six months.
Must be comfortable with driving distances that may exceed 20 minutes between patient visits.

Travel Requirements

Clinicians must be willing to travel up to 75 miles from their assigned care center and be open to on-call rotations.

Expectations

All documentation must be accurate and completed promptly. Clinicians are expected to accept all patients scheduled, regardless of distance, to ensure patient care continuity.
