Job Overview Actify Home Care, a clinician‑owned and operated agency led by a Physical Therapist with over 20 years of experience, is expanding its high‑quality care throughout Palm Beach County. We are hiring skilled Geriatric Occupational Therapists to serve the South Palm Beach Zone, including Boca Raton, Delray Beach, and Boynton Beach. Responsibilities Provide high‑quality, one‑on‑one occupational therapy to geriatric patients in their homes. Complete evaluations, re‑assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Implement plans of care to improve activities of daily living (ADLs) and patient safety. Compensation OASIS rate: $80.00 per visit. Evaluation / Treatment: $65.00 – $70.00 per visit.
