Outpatient Therapist – Bachelor’s/Master’s Level (Mental Health Worker) Program: Outpatient Clinic/School Based Mental Health, State: Pennsylvania

SUMMARY

This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload, ensuring completion of required clinical documentation related to individual caseloads in a timely manner.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Demonstrates proficiency with assessment skills by completing psychosocial assessments at intake and ongoing intervals, completing standardized assessments and/or outcome‑based measures. Develops individualized treatment plans adhering to applicable county, state and federal regulations. Provides therapy to consumers in the recommended setting, following individual consumer treatment plans. Coordinates holistic treatment by collaborating with all team members involved with the consumer. Evaluates individualized consumer‑driven treatment plan reviews, completing ongoing evaluation of efficacy and developing treatment plan reviews. Develops transition and discharge planning by evaluating recovery and progress through treatment goals and symptomology. Completes all required clinical documentation, including progress notes, necessary signatures, discharge summaries, incident reports, and treatment reviews via electronic health record system. Responds to emergency/crisis situations with team members and consumers, ensuring safety and using de‑escalation skills when needed. Maintains assigned schedule following program guidelines to ensure regular service provision. Manages and supports coordination of care through routine conversations and referrals to natural supports, PCPs, school districts, etc. Participates in ongoing supervision, training, department and team meetings as outlined by policies and procedures.

SUPERVISORY RESPONSIBILITIES

This position does not have supervisory responsibilities.

PHYSICAL DEMANDS

May remain stationary 50% of the time; may move as required. May use a computer, copier, printer, and ascend/descend stairs to provide services. Requires frequent communication, routine transport of resources, and may travel up to 100% of the time between appointment sites. Work setting includes home, school, community, clinic, and program office environments.

REQUIRED EDUCATION AND/OR EXPERIENCE

Bachelor’s degree from an accredited college or university in Psychology, Social Work, nursing, rehabilitation, or activity program. Or a graduate degree in a clinical discipline with 12 graduate level credits in mental health or counseling.

PREFERRED EDUCATION AND EXPERIENCE

At least 1 year of experience with a CASSP service system, a licensed mental health services agency, a subcontracted agency, or other mental health clinicians with training in behavioral modification techniques.

QUALIFICATIONS

Language skills: ability to read, analyze, and interpret complex documents; respond effectively to inquiries. Mathematical skills: basic arithmetic and use of Excel. Reasoning ability: make sound decisions, utilize therapeutic modalities, and respond quickly to prevent injury. Proficiency in electronic health system, psychological assessments, teamwork, organizational and communication skills.
