You will work on-site at our Lakeland, FL clinic. You will report to the associate medical director. Join a team that's redefining senior primary care. We are looking for a primary care physician to join our team full-time at Conviva Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.

Main Responsibilities

• You will deliver comprehensive outpatient care to adult and senior patients.
• You will maintain accurate medical records and documentation.
• You will diagnose and manage moderately to complex medical conditions.
• You will work with interdisciplinary teams to ensure holistic care.

Required Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in Florida or to obtain.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine (or board eligible).
• DEA or to obtain.
• One year+ experience working in value-based health care.
• If selected for this role, you will be required to be screened for TB.

Must be willing to participate in on-call coverage by phone.

Preferred Qualifications

• Fluency in Spanish.

Scheduled Weekly Hours: 40

Pay Range: $255,000 - $298,500 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
