Social Service Director Activities Director Dual Role

Job Summary:

We are seeking a highly motivated and experienced Social Service Director/Activities Director to join our team at Cimarron Pointe Care Center in Mannford, Oklahoma. As a dual role, this position combines the responsibilities of both Social Services and Activities Director, providing an opportunity to work closely with our residents, families, and staff to deliver exceptional care and services. Responsibilities:

• Discharge planning, admissions paperwork, care plan meetings, assessments, and interviews with residents

• Arrange transportation and doctor appointments for residents

• Verify benefits for referrals and maintain accurate documentation in PCC

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans

• Coordinate and lead activities and events for residents, including social programs, entertainment, and leisure activities

• Develop and implement programs to improve resident engagement and satisfaction

• Demonstrate a strong understanding of state and federal regulations and guidelines related to long-term care

• Participate in quality improvement initiatives and contribute to the development of best practices

• Provide training and support to staff members as needed

Requirements:

• LTC experience required

• Work Location: In person
