Position Overview

The Cardiovascular Navigator plays a crucial role in coordinating and enhancing the delivery of high-quality clinical care for cardiovascular patients. This position focuses on:

  • Establishing strong relationships with patients and healthcare providers.
  • Coordinating comprehensive care plans, including scheduling appointments, arranging transportation, and providing educational resources.
  • Identifying opportunities to improve care practices by developing evidence-based guidelines in alignment with organizational goals.
  • Acting as a liaison among patients, families, primary care physicians, and various healthcare providers.
  • Conducting outreach and marketing efforts to promote the program.
  • Performing follow-up calls within 72 hours post-discharge to assess patient needs and reinforce care instructions.

This position operates Monday through Friday, with flexible start times before 8 AM. There are no weekend, holiday, or after-hours commitments. As a leadership role, it offers potential pathways to supervisory or management positions within the cardiovascular service line.

Qualifications
  • Graduation from an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
  • Current licensure from the Kansas Board of Nursing is required.
  • Basic Life Support (BLS) certification is mandatory upon hire; Advanced Cardiovascular Life Support (ACLS) certification must be obtained within six months of hire.
  • A minimum of three years of experience in cardiology, demonstrating clinical competence.
