Requirements

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license. Must have reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities

• Provide clinical assessment to identify patient needs and appropriateness of care.
• Document, prepare/review and complete a Plan of Care (POC), progress notes, and other clinical documentation.
• Plan and implement treatment for neuromuscular, neurological, and orthopedic disorders.
• Assist and instruct patients and families on transfer techniques and the use of therapeutic equipment safely.
• Provide patient and family education regarding therapeutic procedures.
• Apply infection control and universal precautions in patient care activities.
• Accept clinical assignments consistent with education and competence.
• Perform other related duties as assigned or requested.
