Develop and update individualized care plans. Monitor client progress and revise care plans as needed. Educate clients and families regarding care plans, treatment, and health maintenance. Observe caregivers in the field to ensure they are following the client's care plan and providing appropriate care. Provide caregiver education and re-education when needed. Communicate with clients, families, caregivers, and other healthcare professionals. Accurately document visits, assessments, and client progress. Maintain compliance with applicable state and federal regulations. Help identify opportunities to improve the quality and safety of client care. What We're Looking For Current Massachusetts Registered Nurse (RN) license. Current CPR certification required. Home care experience preferred. Strong clinical assessment and documentation skills. Excellent communication and interpersonal skills. Compassionate and patient-centered approach to care. Strong attention to detail. Ability to work independently and manage multiple client needs. Comfortable providing education and guidance to caregivers and families. Reliable transportation and willingness to travel throughout the Worcester service area. Why Home Care?
