Urgent Care Licensed Vocational Nurse - Southeast San Diego

LVN Position in Urgent Care Clinic

Under the supervision of the Nursing Supervisor and within established scope of practice, the LVN will provide patient care in the acute care setting of the Urgent Care (UC) clinic, in accordance to delegated duties and scope of practice, and in accordance with San Ysidro Health policies and procedures. The LVN should be able to demonstrate the knowledge and skills necessary to provide individualized care in a manner conducive to comfort, safety, and welfare of patients.

Essential Functions of the Job:

  • Upholds San Ysidro Health's Policies and Procedures, Core Values and competencies and applicable state, federal and local laws.
  • Performs patient care functions as prescribed by licensed health care providers following established UC protocols and policies and procedures within defined scope of education, training, and within defined scope of practice.
  • Follow National Patient Safety Goals and Core Measure Initiatives
  • Perform nursing tasks including but not limited to obtaining vital signs, performing 12-lead EKGs, Intravenous (IV) access insertion, administer PO, IM/SQ and topical medications as per provider's order, and provide patient/family education upon disposition.
  • Perform thermal applications, wound care/dressings, apply orthopedic devices/splints, and urine specimen collection, performs POC testing, autoclave processing of instruments.
  • Assist with transfer within department, set up supplies for specific procedures, Performs Respiratory treatments, visual testing.
  • Displays understanding of standard pharmacological agents used in treating patient conditions.
  • Demonstrate good knowledge and practice of infection control guidelines, sterile techniques, isolation, and universal precautions.
  • Interviews patients, measures vital signs, and other pertinent information as required by UC.
  • Documents pertinent patient information: Data collection of medical history, medication reconciliation, and focused assessment based on chief complaint.
  • Demonstrate principles of aseptic techniques and infection control and prevention measures, as per SYH guidelines
  • Completed orders, nursing procedures, patient's response to treatment, etc.) in the EHR, as established by SYH guidelines and maintaining patient confidentiality.
  • Report any treatment results, observations, patient complaints, or any variation from baseline to charge nurse or provider in a timely manner and modify care plan accordingly.
  • Monitor patient's response to treatment and communicate with other health team members significant changes and modify care plan accordingly.
  • Demonstrate understanding of policies, protocols, organizational structure, and appropriately utilize chain of command
  • Participates regularly in staff development trainings to ensure continuous quality improvements.
  • Always maintain department readiness (i.e. clean/organized work area, restocking, equipment checks, and exam room preparation)
  • Make frequent rounds in the department to maintain tidiness and cleanliness; prepare and decontaminate rooms and equipment between procedures.
  • Applies principles of aseptic technique and infection control as directed by the Infection Control policy and procedures.
  • Assists the physician with therapeutic and diagnostic patient care functions which may require sterile technique, removal, or application of dressings, suturing or wound care.
  • Participates and responds, when assigned to clinical emergency codes.
  • Answer urgent call bell promptly; assist patients if within own scope of practice or report the request/complaint to appropriate personnel.
  • Ensures clinical area is audit prepared by completing monthly Quality Assurance packets.
  • Participates in department performance and Quality Improvement activities.
  • Provides assistance with administrative task (i.e. nursing schedule, supply orders, etc.) as needed or assigned by management.
  • Demonstrate and apply knowledge of safety principles as identified by SYH.
  • Demonstrate knowledge and skills in urgent care service protocols and clinical standards.
  • Perform other related duties as necessary.

  • Practices customer service standards as defined by SYH and specified clinic.
  • Anticipates the customer's needs, takes proactive steps to meet those needs, by listening and taking responsibility within scope to ensure issues are resolved.

Job Requirements

Experience Required:

  • 6 months urgent care experience (past 3 years)

Experience Preferred:

  • 1 to 3 years of experience in acute care setting

Education Required:

  • Graduate of an accredited Vocational Nursing program

Education Preferred:

  • Associate of Science Degree (AS)

Certifications Required:

  • Possession of active valid California LVN license
  • Current IV certification
  • Current BLS (American Heart Association)

Verbal and Written Skills Required to Perform the Job:

  • Ability to read, write and comprehend medical information and terminology.
  • Ability to effectively present information, both verbal and written.
  • Effective interpersonal and communication skills

Technical Knowledge and Skills Required to Perform the Job:

  • Demonstrated ability to use personal computer.
  • Basic computer skills and experience or familiarity with HER
  • Knowledge of legal implications in clinical practice and documentation
  • Demonstrates accountability and professionalism.

Working Conditions and Physical Requirements:

  • Continuous standing/walking/twisting.
  • Occasionally lifts pushes and/or transfers patients.
  • Exposed to unpleasant elements (accidents, injuries and illness).
  • Reliable transportation necessary for traveling to assist with providing coverage as needed throughout all SYH clinic locations.
  • Ability to work a varied, 40 hours per week schedule to include evenings and/or weekends, between the hours of 8 am to 8pm

Equipment Used:

  • Demonstrated ability to use wheelchair, EKG, gurney table, vital machine, pulse oximeter, autoclave.

Universal Requirements:

Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs. Compliance with all mandated vaccinations and all boosters is a term and condition of employment.
