Occupational Therapist (Contract)
Location: Arizona 86404
Pay: $1,987 – $2,230/week
5 shifts per week, 8 hours each (flexible: 4/10s or 5/8s)
Must work until 6:00 PM MST
26-week commitment preferred; permanent possible
Provide occupational therapy evaluations and treatment for patients
Maintain accurate daily documentation using DSCTop and EIEPPRO (for annuals, evaluations, quarterlies)
Collaborate with interdisciplinary team and follow organizational documentation standards
Support patient progress and care planning while adhering to regulatory and safety guidelines
Occupational Therapist license (OT, not COTA) – Required
BLS certification – Required
Experience in patient evaluations and treatment – Preferred
Flexible with schedule and able to work until 6:00 PM MST
