Locums Nurse Practitioner or Physician Assistant

We are seeking an experienced NursePractitioner or Physician Assistant for aLocums at a primary care practice serving geriatric patients inavalue-based care model.This is a hybrid role with blend ofcoverage in outpatient clinics as well as home-based patient care. Compensation: $75 – $95 perhour per day Position Details: Schedule: Full TimeHours Guaranteed Monday – Friday - 5 Days Per Week 8:30 Start Time Patient Load: LOW DAILYCENSUS Home Care: 3–5 patientsper day Clinic: Small census of 7–10 patients perday Setting: Hybrid (Outpatient clinic +home-based care), value-based primary care for elderly patients Responsibilities: Deliver comprehensive primary care services,including preventive care, chronic disease management, and acute caretoGeriatric patients Diagnose, treat, prescribe medications, anddevelop individualized treatment plans Order and interpret diagnostic tests (labs,imaging, etc.) Document accurately in the EHR system Provide patient education and promotewellness initiatives Collaborate with physicians, nurses, andsupport staff to ensure coordinated, high-quality care Refer patients to specialists and managefollow-ups Ensure compliance with state, federal, andprofessional regulations Requirements: Active NP or PA license in NY Minimum 2 years of primary care experience National Board Certification Expedited Credentialing Medicaid and Medicare Provider Id required #J-18808-Ljbffr
