Overview WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. Position Title: EMPOWR Intensive Care Coordinator (ICC) Classification: Regular Full Time (1.0 FTE), Non-exempt (Hourly) Location: Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely) Regular Work Schedule: Full time, M-F Compensation:

• 34.62 per hour
• 38.08 per hour for qualified bilingual Spanish candidates, inclusive of 10% language differential

Program context The EMPOWR program (EMbracing, Progress and Opportunities, We Rise) supports transition age youth (TAY, 16-24) to attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload to provide youth with an entire team of support. Role summary The Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. This position engages and assists foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to address the child and family’s identified goals. The ICC draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help create a connection with family and other close contacts and offer a unique perspective to the rest of the team. Responsibilities Outreach to and engagement with youth; advocate for the youth within the team Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living Provide intensive skill building with youth, focusing on independent living Provide assessments, crisis intervention, stabilization, and safety plans Identify and connect youth & young adults with other agencies to address needs (housing, medical, mental health, education, employment, advocacy, placement stability, enrichment) Collaborate with the team to connect clients to resources and services Assist with initial contact and intake, establishing relationships and building trust to help clients navigate systems Provide advocacy, targeted case management and community-based rehabilitation services Participate in agency meetings in virtual or in-person settings (office, community locations, homes, schools, parks, etc.) Provide stabilization support services in an in-person milieu Qualifications Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or related community resources Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred) Experience providing case management and rehabilitation services At least two (2) years experience working with system-involved youth Ability to work with families who have experienced high trauma and high mental health needs Ability to work independently and collaboratively as part of a multidisciplinary team Strong organizational skills and ability to produce high-quality, timely documentation Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office Clean CA DMV record and vehicle insurance; travel is required Competencies Experience working with children and families who may be involved in the child welfare system Case management skills: assessment/evaluation, crisis intervention, supervision, ethical and legal responsibilities, documentation Cultural humility and awareness of race, class, and ethnicity Time management and ability to complete timely documentation Collaboration and team functioning in a multidisciplinary setting Ability to work with clients with complex trauma histories and attachment challenges Benefits Employer-paid Medical Benefits for Employees 100% employer-paid dental and vision Dependent medical, dental and vision (50% employer-paid) Medical and Dependent Care FSA and commuter plans 100% employer-paid life insurance and long-term disability insurance Voluntary accident, term life and hospital indemnity insurance 403(b) and ROTH retirement plan options, employer contribution targeted at 7.5% after first year Quarterly (5-10%) and annual incentive compensation (10%) Paid time off: three weeks in first year, 4+ weeks with tenure 12 paid holidays plus one floating holiday per year 4 paid self-care days per year Wellness stipend ($100 per month) Professional development stipend and CEUs Employee Assistance Program (EAP) EEO statement WestCoast Children’s Clinic is an equal opportunity employer.
