Job Type Full‑time Position: Nurse/LPN Responsible for providing specialized nursing services to patients and caregivers at home or in alternative settings. Provides technical nursing and skilled interventions as ordered by the physician. Instructs, motivates, and assists patients/caregivers in learning and improving ability to live independently at home. Job Duties Ensures physician orders are followed and included in patient’s plan of care Development and revision of the plan of care based on physician orders Ensures nursing orders are carried out professionally, satisfying ethics, patient rights and regulations Monitors patient outcomes and reports changes to supervisors to prevent readmission Participates in effective communication with other clinicians Completes and submits all required documentation within guidelines Communicates patient progress to supervisor and makes changes under physician direction Covers after‑hours, weekend and holiday on‑call to meet branch needs Maintains confidentiality of company and patient information; protects patient rights as defined in the Bill of Rights Attends staff meetings, required in‑services and other meetings as requested Instructs patients/caregivers on safety measures and implements infection control precautions Ensures accurate patient assessments are completed on time Coordinates related services for patients Responds to emergency patient needs timely, escalating to supervisors and physicians as needed Always represents self in a professional and ethical manner Maintains professional responsibility for self‑protection during hazardous situations Completes all required patient‑care documentation and reports accurately Communicates with clinical staff, physicians, and other health care providers to assure quality and continuity of care Other duties as assigned Requirements Associate degree in nursing or equivalent Current and unrestricted LPN clinical license Two years of clinical nursing experience Five years of related HME experience preferred Valid and unrestricted driver’s license Salary $25.16 – $40.25 per hour AdaptHealth is an equal‑opportunity employer and does not unlawfully discriminate against employees or applicants on the basis of race, color, religion, creed, sex, national origin, age, disability, marital status, veteran status, sexual orientation, gender identity, genetic information, or any other status protected by applicable law.
