Responsibilities
- Provide direct patient care to residents within the licensed scope of practice, focusing on comfort, safety, and quality of life.
- Administer prescribed medications, monitor responses, and document accurately in the charting system.
- Monitor residents' vital signs, assess changes, and communicate findings to the supervising nurse promptly.
- Assist with activities of daily living (ADLs) and support rehabilitation goals as directed.
- Maintain a clean, safe, and infection-controlled environment; adhere to safety protocols and care plans.
- Collaborate with the interdisciplinary care team to coordinate care and optimize resident outcomes.

Requirements
- Active Licensed Practical Nurse license (state of practice to be determined per assignment; Vermont licensure or eligibility for Vermont licensure may be required).
- Graduation from an approved practical nursing program.
- Prior experience in long-term care settings preferred.
- Ability to work Day and/or Evening shifts; able to maintain a 40-hour weekly schedule.
- Willingness to travel and adapt to new team assignments.
