PURPOSE OF THE POSITION : The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs. REPORTS TO : Care Manager Supervisor

SPECIFIC RESPONSIBILITIES The Health Home Care Manager delivers all six NYS Department of Health (NYS DOH) Health Home Core Services , including:
- Comprehensive Care Management
- Care Coordination and Health Promotion
- Comprehensive Transitional Care
- Individual and Family Support
- Referral to Community and Social Support Services
- Use of Health Information Technology to Link Services
- Conducting NYSDOH-required assessments and developing compliant, person-centered Plans of Care
- Coordinating medical, behavioral health, substance use, and social service supports
- Engaging members through outreach, advocacy, and ongoing follow-up
- Collaborating with hospitals, providers, Manage Care Organizations (MCO), and community partners
- Maintaining accurate, timely documentation in NYSDOH-approved systems
- Meeting Health Home productivity, engagement, and quality benchmarks
- Participating in case conferences, audits, supervision, and training
- RELATIONSHIP WITH OTHERS & IN THE WORKPLACE
- Demonstrates professionalism, maintains appropriate boundaries, and follows the agency's

- QUALIFICATIONS
Care Manager must have one of the following:
- A Masters with one year of relevant experience, OR
- A Bachelors of Arts or Science with two years of relevant experience,
- A License as a Registered Nurse with two years of relevant experience, OR
- Complete Children Adolescent Needs Assessment (CANS)NY training
- Be CANS-NY certified annually
- Meet exam score requirements:
- Care Managers: minimum 70%
- Supervisors: minimum 80%
- Bilingual Preferred ( Spanish )
