Occupational Therapist (OT) Home Health Full-Time /PRN

Job Details Location: Baytown, Highlands, Channelview, San Jacinto, Galena Park, TX. Employment Type: Full Time & PRN. Schedule: Flexible daytime schedule. Work Setting: Home Health / In-Home Physical Therapy. TheraCare is seeking a licensed Occupational Therapist (OT) for a Full-Time or PRN home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Baytown, TX and surrounding areas. Job Responsibilities Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients. Develop and update individualized Plans of Care (POC). Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions. Address ADLs, IADLs, coordination, strength, balance, and functional mobility. Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments. Implement interventions for fall prevention, home safety, and environmental modifications. Train patients and caregivers in adaptive equipment and durable medical equipment (DME). Fabricate splints and recommend assistive devices as appropriate. Complete accurate and timely documentation in the EMR. Communicate with physicians and interdisciplinary care teams. Supervise COTAs and home health aides, as applicable. Follow all home health regulations, infection control, and safety standards. Benefits Competitive Compensation Package. Mileage Reimbursement. Active Occupational Therapist (OT) license in the state of Texas or eligible Compact OT Licensure. Home Health experience (1 year required). HomeCareHomeBase (HCHB) experience (preferred). Current CPR certification. Strong communication, clinical judgment, and time-management skills. Knowledge of home health regulations and Medicare guidelines preferred.
