Home Health Registered Nurse

Responsibilities Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments Develop and implement individualized care plans with physicians and other healthcare professionals Monitor patients' conditions and report changes Educate patients and families on disease management, medication, and treatment options Maintain accurate patient care records and coordinate with healthcare professionals Report patient care and condition progress to the patient's physician and Clinical Manager Oversee Home Health Aides and monitor completion of their documentation Requirements Med surg, ICU, ER, or acute experience Current and unrestricted Registered Nurse licensure Current CPR certification Experience collaborating with a team of healthcare professionals Valid driver's license Auto insurance Reliable transportation One year of nursing experience preferred Home Health experience preferred Must complete TB screening if selected Must maintain personal vehicle liability insurance according to state requirements or specified minimum limits Core Competencies Demonstrates expertise in providing skilled nursing care, including medication administration, wound care, and patient assessments.
