Acute Care Medical Social Worker

This is a 13-week contract position for an Acute Care Medical Social Worker. The role involves providing care management and discharge planning for patients in an acute care facility. The Medical Social Worker will work collaboratively with an interdisciplinary team to ensure effective discharge processes and address post-acute care needs for a diverse patient population.

Responsibilities

• Conduct initial assessments within 24 hours of patient admission.
• Coordinate discharge plans and facilitate connections to post-acute care services.
• Document care delivery, plans, and patient education based on the established protocols.
• Work closely with medical teams, insurance case managers, and other stakeholders to ensure safe and timely discharges.
• Perform bio-psychosocial assessments and develop individualized care plans.
• Educate patients and caregivers to facilitate successful discharges.

Required Experience / Certifications / Licensure

• Must possess a valid LCSW or MSW.
• At least 1 year of acute care hospital experience is required.
• Experience with Epic charting system is required.
• Previous experience in care coordination and case management is preferred.
• Must be willing to work a variable schedule including weekends, with days off not guaranteed to be consecutive.

Schedule

Day shift, 5x8 hours from 08:00 AM to 04:30 PM. Weekend requirements may vary based on departmental operations.
