Oasis Therapy Services is seeking a Physical Therapist to provide one-on-one rehabilitation services in Apple Valley, Minnesota. Available employment option: Part-Time EMR: Net Health Responsibilities Evaluate residents and develop individualized physical therapy treatment plans. Provide treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention. Monitor residents’ progress and modify treatment plans as needed. Complete discharge planning and provide recommendations for continued care. Educate residents, families, and caregivers on exercises, safety, and assistive equipment. Supervise Physical Therapist Assistants when assigned. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Minnesota Physical Therapist license. Graduate of a CAPTE-accredited Physical Therapy program or a program that meets CAPTE requirements. Current CPR certification. Strong communication, organization, and documentation skills. Geriatric, senior living, or rehabilitation experience is preferred. Why Join Oasis?
