Job Description Summary This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at the assigned hospital. It is responsible for providing services to patients, families and significant others, including completion of biopsychosocial assessments, psychiatric diagnoses, formulation of treatment plans and treatment interventions, therapy sessions (individual, couple, families or group), implementation of treatment plans and discharge planning to support and/or enhance biopsychosocial functioning. The role receives referrals for individuals from at‑risk populations from interdisciplinary team members such as physicians, case managers, staff nurses, and other members of the care team. Must demonstrate knowledge and skills necessary to provide appropriate care. Must understand principles of growth and development across life cycles. The position serves specific age groups and requires the ability to act quickly and work in high‑stress situations. Responsibilities and Duties Clinical Assessment – 30% : Conduct patient/family/interview, chart review, and professional collaboration to interpret biopsychosocial information; develop care and discharge plans; identify abuse, neglect or exploitation cases and refer them to mandated authorities; provide crisis intervention, information, referral and linkage to community resources. Intervention Management – 30% : Develop intervention plans based on assessment to resolve problems and enhance functioning; use individual, family, or group modalities; employ clinical intervention skills such as treatment planning, continuum of care planning, supportive counseling, crisis intervention, problem‑oriented planning, and educational counseling. Population Management – 25% : Coordinate clinical and financial outcomes of an assigned population; collaborate with interdisciplinary staff and external organizations for timely, cost‑effective patient care; monitor quality and type of services; act as liaison with payers for post‑discharge finances; ensure completeness of treatment plans and reports. Documentation and Regulatory Compliance – 10% : Document assessments and interventions in the patient’s medical record and information systems in accordance with department standards; comply with internal and external documentation requirements and regulatory obligations. Other Functions – 5% : Maintain awareness of hospital, community services and appropriate care options; provide information and facilitate linkages between these services and the patient/client; perform additional functions as appropriate. Minimum Qualifications Master’s Degree in Social Work (Required); DL‑HC driver’s license only if in a Home Care department; LSW – Licensed Social Worker; Social Work Certification and Licensure Board. Masters degree in Social Work or a related field and LSW license in the State of Ohio. 1–2 years’ experience as an LSW, preferably in a patient‑centric setting and knowledge of medical terminology and computer skills. Licensed Social Workers with a Bachelor of Social Work hired before 01/10/2019 are grandfathered into this job; those hired after 01/10/2019 must meet current qualifications.
