Hospice Registered Nurse Case Manager

Hospice RN Case ManagersHospice Services of Massachusetts is seeking per diem Hospice RN Case Managers to join our team of caring professionals. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care as an RN, building from the resources of the community to meet the needs of individuals and families within their homes and communities.$42 to $52 an hour, depending on experience, with weekly pay.Responsibilities:Supervise hospice aidesComplete comprehensive assessments of patients and families to determine hospice needsProvide professional nursing care by utilizing all elements of the nursing processAssess and evaluate patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessaryInitiate the plan of care and make necessary revisions as patient status and needs changeDevelop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actionsProvide health care instructions to the patient as appropriate per assessment and planExperience & Education:Graduate of an accredited school of nursingOne (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home careCurrent licensure as an RN in Massachusetts and CPR certificationBachelor's degree, with one (1) year of hospice care experience preferredWhat we offer:Competitive and Weekly PayHoliday Pay for Hourly and Salaried EmployeesMileage ReimbursementOvertime Pay for Hourly EmployeesCareer Advancement OpportunitiesExclusive Employer Discount ProgramAvailable for Eligible Team Members:Employer-Paid Life Insurance401(k) with Employer MatchPersonal DaysVacation and Personal TimeHealth, Dental, and Vision InsuranceWe are an equal opportunity employer that values diversity at all levels.
