Team 7 Covers Northern Suffolk; Centereach, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jefferson Station, Setauket, South Setauket, St. James, Stony Brook, and Terryville. * 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends * Great Healthcare benefits, and education reimbursement of $7,000 annually * Additional compensation for visits over your caseload Job Details Our Home Care RN's: * Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. * Measure vital signs, Administer IVs, change dressings, clean wounds as needed * Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. * Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. * Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment. * Provide care to the most upmost standard Minimum Qualifications: * New York State RN license * Minimum of one year of recent hands on experience as a Registered Nurse. * Basic Life Support Certification * Valid New York State Driver's License and reliable transportation Salary Range USD $95,000.00 - USD $125,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
