Wellness Coordinator

Supporting underserved teammates, the remote Wellness Coordinator - Louisiana/Texas will provide community support services, assist with resource navigation, and conduct needs assessments, while being based in either Louisiana or Texas. Key responsibilities Conduct and analyze needs assessment data to identify social determinants of health and appropriate community resources Utilize Christus Health's LiveWell platform to connect team members with necessary support and resources Build and maintain relationships with community partners and communicate with department managers to ensure proper support for team members Required qualifications Bachelor's degree required, preferably in Health Education; MSW or Certified Health Coach preferred Two years of experience in healthcare required Ability to communicate effectively, both verbally and in writing Willingness to be onsite to support all shifts, including occasional weekends and nights Strong organizational skills to manage detailed tasks under time constraints
