Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care Advanced Practice Provider (APP) for our clinic in Pompano East, Florida. You will have the opportunity to take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. As a Primary Care Physician, you will report to our Associate Medical Director.

Key Responsibilities

• Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
• Manage chronic conditions with an emphasis on preventive care and wellness
• Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
• Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
• Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
• Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Participate in care team meetings and quality improvement initiatives

A day in the life of a Conviva APP

• Monday-Friday 8:00am-5:00pm
• Join an experienced team with an established patient panel
• See 12-15 patients per day. Opportunity to spend more time with patients
• Longer authorized appointments (45 minutes)
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care

Qualifications

• Active, unrestricted Nurse Practitioner license in the state of practice
• National certification (AANP or ANCC)
• DEA license (or ability to obtain)
• Minimum of two+ years of experience
• 1+ years of experience managing Medicare Advantage panel of patients
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Fluent in Creole and English

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
