Hospice Admissions Nurse — Compassionate Care Specialist

Longleaf Hospice and Palliative Care in Atlanta, GA seeks an Admission Nurse to perform consultation and admission visits, assess patient/family needs, and determine hospice eligibility with physicians. This role involves coordinating with the IDG, developing care plans, and providing education on Hospice services.
