What You’ll Do Complete an initial, comprehensive and ongoing assessments to determine home health and/or hospice needs. Develop patient focused care plans that establish goals, based on nursing diagnosis and incorporate palliative nursing actions. Provide respectful, compassionate care in patients’ homes. Act as Case Manager and assume responsibility to coordinate patient care for an assigned caseload. Collaborate with interdisciplinary team members. Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician’s orders as required. What You Bring Active RN licensure (state-licensed). Valid driver’s license, reliable transportation, and current auto insurance. Strong communication skills and a team-oriented mindset.
